Новая форма документа представляет собой выписку из Единого регистра застрахованных (ЕРЗ). Выписка содержит сведения о застрахованном лице, номер полиса ОМС, штрих-код, контакты страховой компании.
Если вы потеряли пластиковый или бумажный полис, необходимо: ✔️ Обратится в свою страховую медицинскую компанию, где вам в течении 5 мин сформируют цифровую копию полиса ОМС в виде выписки из ЕРЗ, которую можно скачать или распечатать на бумажном носителе.
✔️ Получить данные на сайте Госуслуги. Выберите вкладку «Документы», затем – «Здоровье», «Получите данные о своём полисе ОМС» и нажмите кнопку «Запросить».

До 31 декабря 2025 года действует переходный период: медицинские учреждения, у которых пока нет технической возможности получить доступ к ЕРЗ, могут попросить предъявить полис ОМС на любом носителе: бумажном, пластиковом, в виде выписки с данными штрих-кода.
