Cet ouvrage est autant un essai théologique sur l’orthodoxie religieuse qu’un témoignage personnel et spirituel, intégrant de nombreux éléments autobiographiques. Orthodoxie ne désigne en effet aucunement les églises orientales dites « orthodoxes » ; c’est de l’orthodoxie comme concept qu’il s’agit, celui de la croyance, de « l’opinion droite » qui s’oppose aux opinions fausses. Dans l’histoire du catholicisme, ce terme désigne la fidélité aux croyances traditionnelles telles qu'elles ont été définies par les Écritures, les premiers conciles de l'Église et la tradition apostolique. Chesterton utilise ce terme comme une revendication provocante, car il défend une conception de l'orthodoxie chrétienne souvent attaquée ou mal comprise à son époque, notamment par les courants rationalistes, matérialistes ou modernistes.
