برخی مبتلایان بدون آنکه علائم واضح ابتلا به آبله میمون را پیدا کنند ناقل این بیماری بوده‌اند، مثلاً برخلاف تصاویر رایج کتابها و رسانه‌ها بثورات پوستی روی دست و صورتشان ظاهر نشده و تنها در بخش محدودی از بدن بوده است و/یا برخی از علائم معمول در آبله میمون را نداشته‌اند، با این حال ناقل بیماری بوده‌اند؛ این عامل هم سبب می‌شود که کنترل این بیماری سخت‌تر از حدی که تاکنون تصور می‌شده باشد.
