Here's a lesson for ya. In the basement of a paper machine there is a "rail car" used to move long items lowered thru a hole in the operating floor from teh machine to near the roll turning lathe room. I say rail car in quotes because it does ride on rails but it is not meant for huge loads and runs off a small 5kw LP powered onboard generator and a small VFD drive. It has a corded pendant that was originally about 12' long. Over the years that cord was run over a few times and cut... so it got shorter and shorter. by now it's about 2' long and you have to walk very closely beside this thing as you drive it. it moves past a tank dyke curb with a gap too small for a man's foot. So as you walk you just step up onto the dyke and keep walking. Well they let a women drive it. She stepped between the rail car and the concrete dyke and the rail car crushed her foot horizontally while anohter part of it protruding tore up her ankle.

There are a few lessons here 1) maint guys need to analyze why that cord got cut and come up with some way to make sure it doesnt happen again other than shortening it. Maybe a pipe sticking up to suspend the cord from?? 2) when you train someone to do a job don't train them only on what TO do... train them on what NOT to do. Show them the gotchas. it's natural to focus on teaching how do get a job done though.
