Знак будет выполнен в виде серебряной четырехлучевой звезды, в центре которой изображен портрет Великой Княгини Елизаветы Федоровны Романовой. На обратной стороне награды будет размещена надпись: «Помогать нуждающимся. Е.Ф. Романова».
